Peer Support Platform Extends Care Continuum to Help Patients Improve Health Outcomes
Solana Beach, CA — September 17, 2013 – Viverae Health® Solutions, Inc., the leading behavior change platform company improving health outcomes and the cost of care for patients, providers, payers and employers, today announced that the Institute of Physical Medicine and Rehabilitation, IPMR, a non-profit comprehensive medical rehabilitation center providing inpatient and outpatient therapy at 18 locations throughout central Illinois, has selected Viverae Health to provide a peer support platform to engage patients between visits, help integrate cancer support into services and elevate the level of care and connectivity provided to patients.
The contract expands Viverae Health’s market position in the healthcare provider segment and supports growth of the platform’s condition-specific medical communities.
IPMR will offer patients 24/7 access to Viverae Health’s private, HIPAA-compliant online and mobile Social Solutioning® platform providing access to peer-based health communities that engage patients through emotional check-ins, evidence-based tools and game mechanics to help better manage their care and sustain positive health outcomes.

IPMR’s range of services includes 63 separate therapies in addition to its Survivorship Training and Rehabilitation, STAR Program®, which provides critical physical and psychological rehabilitation to cancer survivors by a team of STAR Certified™ clinicians and providers. Viverae Health’s platform will be included as part of the STAR Program, where users will contribute to the growth of Viverae Health’s cancer community.
Viverae Health’s platform also provides a dedicated community for IPMR’s providers, where they can offer patient education and tools in support of the STAR Program that help patients stay on track with cancer rehabilitation goals.
The platform also provides another layer of support to caregivers that use IPMR Senior World adult day service for their loved ones.
